Client Identity Services within the Department of Social Protection
Requirements
For adults (18+):
– Completed application form (GRC1)
– Form must be witnessed by:
- Peace Commissioner, or
- Notary Public, or
- Commissioner for Oaths, or
- Solicitor
For ages 16-17:
– Same requirements as adults
– May need exemption from Circuit Court
Procedure
Complete GRC1 form from Department of Social Protection website, witnessed by authorized person
Upon approval, can request revised birth certificate
Decision Type
Administrative decision
– Can be appealed to court within 90 days
– Court may extend appeal period for good reason
