Requirements
– Sexologist’s diagnosis confirming gender identity disorder – Courts may require additional forensic expert opinions
– Proof of permanent sense of belonging to the identified gender
– Absence of mental illness and hormonal or genetic abnormalities
Procedure
– Civil lawsuit must be filed against parents
– Court examines permanence of gender identity
– After court ruling: Application to civil registry office for new PESEL number; Amendment of birth certificate (gender change remains visible)
– Decision on new first name and potentially surname
